Our care Management team consists of dedicated and caring health care professionals involving registered nurses (RNs), licensed practical nurses (LPNs), licensed social workers and community, and their healthcare providers, to coordinate care toward meeting each person’s health and wellness goals. The quality care coordination we provide includes patient education focused on preventive care and disease management, navigation the healthcare system, as well as identifying and implementing community supports and resources. Our services result in better outcomes, lower costs, and improving experiences for both the patient and healthcare provider.
