Our Population Health Management team seeks to improve health outcomes, reduce costs and increase quality for our patients throughout the continuum of care. We achieve these goals through care coordination programs, data analytics, quality initiatives, pharmacy initiatives, and close collaboration with our providers. Our team works closely with acute care hospitals, post-acute facilities, ambulatory providers, and community partners to improve health care delivery and patients’ healthcare experience.
