The Quality Performance team works with Patients, Provider Practices, and Payors to help close quality gaps and improve performance.

For Patients

The SoNE HEALTH Quality Performance team works with patients to establish care with a Primary Care Provider (PCP), either helping to schedule an appointment with their existing PCP or helping them find a PCP if a patient doesn’t currently have one. We can also assist with coordinating and encouraging patients to get their preventive screenings or lab tests, either by providing education, answering questions, obtaining a referral, or scheduling. The goal is to encourage patients to receive routine preventive care with their PCP and follow through with recommended screenings.

For Provider Practices

SoNE HEALTH provides performance data and patient registries to our practices so that they can prioritize patient scheduling and address any quality gaps in care. The Quality Performance team now offers concierge support services to our Primary Care practices to optimize their office workflows and is tailored to meet the office’s needs. The goal is to engage with the practices and collaborate on continuous improvement activities to maintain high performance.

For Payor Partners

The SoNE HEALTH Quality team facilitates chart audits and record collection as required by our payor partners to close certain quality gaps.  We also work with payors on patient outreach initiatives and at-home screenings, when available, to promote overall patient health and wellness.
