Our Quality Performance Team members are aligned to SoNE HEALTH providers. They work closely with the practices to provide Concierge Services and Improvement Plans. The Quality Performance Team works to establish a regular meeting cadence with offices to share performance, develop action plans and provide education, toolkits, and quick reference guides for quality measure definitions and best practices. This team facilitates the identification of patients that are in need of preventative screenings. They are subject matter experts in HEDIS Quality Metrics. In addition to working with our providers, they also perform outreach to our patients to assist with scheduling preventative services, i.e. mammograms.
They are committed to identifying and closing gaps in care in accordance with HEDIS. HEDIS measures address a broad range of important health issues such as medication use for chronic conditions, cancer screenings, childhood, adolescent, and adult immunizations, care of patients with chronic conditions, and safety and mental health screenings. The Quality Performance Team will review medical records to extract and submit pertinent data to close these gaps in care. If data cannot be found, the QDS team will identify patients who need intervention to close the gap and work with providers to order appropriate tests/screenings.

Manager of Quality & Performance, Clinical Integration
Tracy Reilly has received her BS in Leadership and Management from Albertus Magnus College and her MS in Legal Studies with a concentration in Applied Research and Healthcare. Tracy has held various roles within the healthcare industry throughout her career on both the provider and payor side. She is a solutions-driven professional with an extensive background in quality, population health management, practice transformation, operation management, and project management. She brings her vast knowledge of program deployment and management of healthcare transformation initiatives with her from the last 15+ years on the payor side. She most recently comes to us from Anthem where she was the Medicare Quality Program Manager and was responsible for Medicare Quality Performance and Risk Adjustment for Medicare providers in VBC agreements.
Quality Data Coordinator
Morgan has been with SoNE since 2021. She is passionate about quality metrics, sharing best practices, and collaborating with our providers. She is pursuing her education in nursing to further her career. Her goal is to work with patients and enable them to become active participants in their healthcare journey.
