The Bundle Payment Care for Improvement Program focuses on care management based on patient diagnosis and Bundle Payment plan with goal to reduce risk of utilization, rehospitalization. RN Care Manager Bundle Payment Specialists work with identified patients for 90 days that includes collaboration with the inpatient care management team while patient hospitalized for optimal transition of care.
